.5 FTE/20 Hours per Week



The Pharmacist - Clinical is responsible for providing professional and quality pharmacy services in the GHC-SCW Health Center Pharmacies and clinics in compliance with applicable laws and regulations pertaining to the practice of pharmacy. The incumbent is also responsible for maintaining competency through continuing education and promoting of health education among patients and staff and maintains one of the specific multi-disciplinary or pharmacist certifications listed in this Job Description. The Pharmacist - Clinical is responsible for providing excellent customer service in the performance of all duties. The Pharmacist - Clinical works under the general direction, supervision and guidance of one or more of the following: Managing Pharmacist, Assistant Managing Pharmacist, Pharmacy Operations Coordinator, and the Clinical Pharmacy Program Manager.



JOB SPECIFIC MINIMUM QUALIFICATIONS



  1. Valid license to practice Pharmacy in the State of Wisconsin required.
  2. Maintains one of the following specific multidisciplinary certifications from the organization referenced: Certified Asthma Educator (www.naecb.org), Anticoagulation Care (www.ncbap.org), Clinical Pharmacology (www.abcp.net), Certified Diabetes Educator (www.ncbde.org), Advanced Diabetes Management (www.diabeteseducator.org), Lipids (www.lipidspecialist.org).
  3. OR, maintains one of the following specific certifications from the Board of Pharmacy Specialty (www.bpsweb.org): Ambulatory Care, Cardiology, Geriatric, Infectious Disease, Nutrition Support, Pediatric, Pharmacotherapy, Psychiatric Pharmacy.
  4. Qualified certifications are limited to those in items (2) and (3) unless previously approved by the Clinical Pharmacist Program Manager and the Manager of Pharmacy Services.
  5. Maintains current CPR certification.
  6. Minimum of one (1) year of experience as a Pharmacist preferred.
  7. Excellent customer service skills required. Strong interpersonal skills. Ability to be culturally sensitive and work with a diverse patient population.
  8. Ability to effectively communicate and consult with patients, practitioners and internal staff required.
  9. Excellent verbal communication and listening skills both in-person and on the phone required.
  10. Knowledge of interviewing practices and techniques used to gather information.
  11. Ability to maintain quality, safety and/or infection control standards.
  12. Knowledge of or ability to learn and use computer based applications such as MS Office, prescription processing software and electronic medical record systems such as EPIC.
  13. Ability to remain current on new trends, pharmaceuticals, pharmaceutical related treatments, disease state management, clinical outcomes and other technologies.
  14. Basic understanding or ability to learn about pharmacy reimbursement/insurance practices.
  15. Ability to work as a member of a team and to maintain good working relationships with patients and staff required.
  16. Ability to pay close attention to detail and problem solve. Good organizational skills.
  17. Ability to maintain patient confidentiality required. Knowledge of HIPAA requirements.
  18. Ability to remain calm and be resourceful in emergency situations required.
  19. Ability to maintain a professional demeanor at all times.
  20. Ability to float to other pharmacy or clinic locations, as needed. Ability to provide own transportation to various locations, as required.
  21. Ability to bend, reach, lift, pull, push and/or carry up to 20 lbs. regularly required.
  22. Ability to stand for prolonged periods of time required.
  23. Ability to use a keyboard extensively required.
  24. Fine manual dexterity required.
  25. Ability to see near, mid and far range required.
