SUMMARY
The Manager, Care Coordination oversees daily care coordination operations, ensuring departmental goals are met while developing team members and fostering overall team effectiveness.
We are seeking a strategic leader who will build, implement, and oversee strategy to improve health outcomes for transitional care and complex care patient populations. Experience in population health, interdisciplinary health care, quality, care management, case management, value-based care, strategy design and execution, process improvement, data analysis, and health equity preferred.

Duties & Responsibilities

1. Develops annual work plans used to achieve goals and performance metrics.
2. Directs and manages the daily care coordination operations of the department to ensure policies, procedures, and guidelines are being followed.
3. Participates in leadership activities for division, contributing to the achievement of goals and objectives; evaluates team performance. Defines and maintains internal processes for function; establishes patient experience standards and handles operational and patient/family escalations for function. Serves as a champion of enterprise-wide culture, influencing culture change to support functional objectives.
4. Provides clinical expertise and leads department team members in providing developmentally appropriate care and evaluating outcomes for infants and children with complex health care needs.
5. Utilizes expert evidence-based practice to provide clinical expertise to the healthcare team in coordinating developmentally appropriate care and evaluating outcomes for patients with complex health care needs. Provides clinical follow up when desired outcomes are not achieved.
6. Manages communications to the enterprise on projects and important topics related to nursing care coordination. Maintains effective working relationships with peers in division, peers in other divisions, and senior leadership.
7. Assigns and reviews work, provides work instruction (OTJ or formal training) and develops team members. Evaluates performance of team members, providing continuous feedback. Manages discipline and performance issues; hires and fires team members. Identifies resource needs and develops recommendations for meeting needs.
8. Oversees team spending and ensures alignment with departmental budget, approving expenditures in accordance with organizational policies and demonstrating accountability for fiscal stewardship.
9. Works on highly complex or controversial projects; performs project management activities. Provides technical expertise on programs, processes, and best practices.
10. Ensures compliance against federal, state, and local regulations for functional area.
Minimum Qualifications
Education: Master of Science in Nursing (MSN) or Master's in health related field.
Experience: Three (3) years of clinical experience and two (2) years of nursing leadership experience in supervision, leading clinical practice projects, leading care coordination, and/or precepting. Prior experience with Population Health and/or Care Coordination preferred.
Equivalency: Bachelor's degree in nursing (BSN) plus two (2) additional years of leadership experience over nursing or nurse care coordination will substitute for the Master's degree requirement.
Certification(s): Magnet recognized certification in care coordination, case management or related specialty field required.
Licensure(s): Registered Nurse License
Salary Information
Pay is dependent on applicant's relevant experience.
Annual Salary Range (Based on 40 hours worked per week): $112,925.52 to $169,388.27
