Responsibilities

Hours for this role are Monday - Friday, 8a-430p, No Weekends Required!

Are you ready to make a profound impact in the lives of young individuals facing mental health challenges? At our 101-bed Psychiatric Hospital in Brooklyn Park, Minnesota, we are seeking a dedicated Inpatient Social Worker to join our dynamic team. In this vital role, you will carry a patient caseload, working with adolescent and young adult patients aged 5 to 35 who have an average length of stay of 7-10 days. Your expertise will be crucial in facilitating group therapy, develop treatment and discharge plans, and coordinating care with families, caregivers and the community.

What You'll Do:

As an Inpatient Social Worker, you will be at the heart of our treatment efforts, collaborating daily with Psychiatrists and Therapists as part of the primary treatment team. Your responsibilities will include:

  • Uphold and apply clinical knowledge and understanding of emotional and behavioral health practices and principles
  • Assess, plan, implement, and evaluate the psychosocial services for patients and share social work expertise with members of the treatment team within the average length of stay of 7-10 days
  • Develop and revise treatment and complex hospital discharge plans and communicate these with patients, caretakers, the treatment team, and direct care staff
  • Provide psychoeducation to patients, caretakers, the treatment team, and direct care staff regarding diagnosis, symptom management, recommendations, discharge plans (including leading discharge meetings), safety plans, patient rights, financial impact issues, and other pertinent patient care matters
  • Communicate effectively with external providers, agencies, and government and private payer networks to develop relationships and refer patients to the next appropriate levels of care following hospitalization
  • Lead child, adolescent, and young adult therapy/process groups on the inpatient units
  • Within the training process, complete Think: Kids: Collaborative Problem Solving (CPS) training and recognize that Kids Do Well If They Can. Participate in using the CPS Assessment and Planning tool to help kids develop skills for their challenging behaviors.
  • Oriented to ethically provide and advocate for patient-centered, culturally appropriate, age-appropriate care

Qualifications

  • Requires a Master's Degree in Social Work. Requires licensure in the State of Minnesota, LGSW required, LICSW preferred.
  • Two years' experience in a mental health care setting preferred.
  • Experience working with children, adolescents and their families strongly preferred.
  • Experience using electronic health records preferred.
  • Knowledge of Microsoft Office products including Word, Outlook, and Teams.
  • Ability to communicate in written and verbal form effectively and efficiently.
  • Ability to coordinate care with relevant staff.
  • Ability to shift between dissimilar tasks smoothly and rapidly.
  • Ability to make effective decisions while in stressful situations and circumstances.

Compensation for this role ranges from $30-$41 per hour depending on licensure and experience.
