JOB DESCRIPTION: SUMMARY OF POSITION This position requires traveling within the following southern counties: Alexander, Pulaski, Union, Williamson, Jackson, Johnson, Massac, and Franklin. The role is responsible for providing wrap-around and/or intensive care coordination to youth and families with high needs. Key responsibilities include conducting in-person, community-based child and family team meetings, reviewing IMCANS, supporting crisis services, and evaluating prevention and safety plans. The position works collaboratively with other service teams to meet the needs of clients assigned through Pathways and may involve assisting with connecting clients to appropriate services, advocating on their behalf, providing ongoing support, and monitoring the effectiveness of services.
