This role works closely with physicians, nurses, and other healthcare professionals to ensure safe, effective, and evidence-based medication therapy for our patients. The Clinical Pharmacist serves as a vital drug information resource and contributes to optimal patient outcomes through collaborative care and clinical expertise. This position performs duties in a manner consistent with our mission, values, and Mercy Service Standards.Primary Responsibilities: 
 Mandatory Responsibilities:Review patient records to assess appropriateness of medication therapyEvaluate patient conditions to ensure comprehensive treatmentDevelop medication plans that minimize adverse effectsConsult with prescribers, nurses, and patients regarding medications and dosingAdvise on proper drug administration and monitor treatment outcomesCollaborate with healthcare professionals to optimize patient careMaintain accurate documentation of medication plans and patient progressRoutinely consult with medical staff regarding drug orders, side effects, and formulary considerationsStay informed on hospital and departmental policies and proceduresMaintain professional relationships with medical and nursing staffAttend relevant conferences and maintain licensure requirementsServe as a clinical resource for care center staff with subspecialty expertisePractice Medication Therapy Services including patient assessment and therapy recommendationsParticipate in committees and department initiatives as requestedEnsure compliance with regulatory, licensing, and accreditation standardsContribute to drug utilization evaluations and adverse drug reaction reportingSupport education of pharmacy students and residents 
 Technology Proficiency:Complete expiration date checks and inspectionsPerform pharmacy and unit floor stock checksFulfill mandatory fire/safety training and continuing education requirements 
 Professionalism & Service Standards:Verify orders and document clinically using Epic or similar systemsUtilize drug information software (e.g., Micromedex)Operate pharmacy automation systems (e.g., Omnicell, McKesson, ScriptPro, Baxa) 
 Qualifications:Demonstrate Mercy Signature Service communication skillsExhibit professionalism and courteous behavior with coworkers and patientsMaintain proper hygiene, PPE use, and dress code complianceWork efficiently under time constraints and meet hourly deadlines Education: 
 Minimum: Bachelor of Science (BS) or Doctor of Pharmacy (PharmD) from an accredited school of pharmacy Licensure: 
 Current pharmacist license in the state of practice Other Requirements:
