RN Clinical Coordinator

Our care community is in search of an RN Clinical Coordinator for our Special Care Unit (memory care) neighborhood. We are looking for a compassionate, experienced professional who will monitor nursing care activities for the neighborhood under the supervision of the Director of Nursing. The Clinical Coordinator has the responsibility to perform administrative and supervisory duties for the neighborhood, and to create and complete care plans and assessments for the residents, as well as taking an active role in the direct care of our residents. We're looking for an individual who truly cares about making our community the best home possible for our residents.

Full-Time management-level position (40 hours/week). On-Call Rotation. Flexible Schedule.

MAJOR JOB FUNCTIONS:(include, but not limited to)

• Receives information daily on residents
• Evaluates residents with change in condition and gives input to nursing staff regarding plan of care as needed
• Assures residents receive prescribed medications, diets and treatments
• Assures residents are comfortable, clean and well groomed
• Assures residents' reach highest level of independence by providing rehabilitative nursing care and training in self-care whenever able
• Assures incident reports are complete, investigated, and reported to regulatory authorities as needed
• Assists with referrals and admissions as needed
• On-call requirement

Qualifications and skills:

• Possess a current, unencumbered Minnesota RN.
• Preferred 2 years of experience in Long Term Care
• Preferred, but not required, two years of nursing management experience
• Ability to interact with a variety of people and promote a team philosophy
• Demonstrate leadership abilities to develop and motivate people
• Good critical thinking and problem solving skills

The Lutheran Home: Belle Plaine is a 75-bed skilled nursing facility that comprises of a Transitional Care Unit (TCU), Long-Term Care (LTC) and memory care.
