Requirements

The ideal candidate will possess the following qualifications:



  • Graduate of accredited LPN nursing program.
  • Current license as LPN in Commonwealth of PA.
  • Must provide proof of LPN certification renewals.
  • Must be able to work with adults with mental disorders preferred.
  • Must have minimum of one year experience with adults preferred.
  • Must attend ongoing trainings with prior notice
  • Must have a valid driver's license and clean driving record.
  • Available shifts are: Every Friday 7am -11pm, and every other Sat/Sun: 7am-11pm

We are seeking a Licensed Practical Nurse (LPN) to join our team at our residential program to work PRN hours at our 16-bed Personal Care Home in Bethlehem, PA.




Under the guidance of the Program Director the LPN will provide services to clients being served in the Personal Care Home (PCH) Program.


Responsibilities include but are not limited to the following:



  • Understand and implement Merakey's Lehigh Valley Center policies and procedures relating to daily routines, client care and rights, safety management, and unusual incidents.
  • Complete assessment forms on each client being seen for medication injections medication groups and second generation anti-psychotic medication tracking.
  • Complete discharge summaries.
  • Contact individual pharmacies to authorize medication refills on any client related issues.
  • Role model de-escalation techniques and best practices to Psych techs and Peer support.
  • Eork as a full member of the PCH team to provide direct care and support to the individual residents.
  • Embrace sanctuary principles in managing the PCH.
  • Promote community collaboration and establish linkages with individual agencies in the community that will be essential to the PCH resident's recovery.
  • Administer medication and therapeutic treatments as needed, as per physician's orders. Monitor for potential side effects and report it to physicians.
  • Provide care in a manner that considers the patients rights, safety, comfort, and the overall therapeutic environment.
  • Observe and document and report resident behavior
  • Participate in continuous quality improvement.
  • Communicate with co-workers and supervisors in timely manner of situations that affect patient care or well-being.
  • Assist in the orientation/training of new staff members.
  • Understand and implement Merakey PCH policies and procedures relating to daily routines, client care and rights, safety management, and unusual incidents.
  • Coordinate, attend, and facilitate follow up from Doctor's appointments.
  • Coordinate preventive care appointments for all residents.
  • Update or complete and communicate with staff, support plans consistent with doctor orders and appropriate care.
  • Help coordinate admissions of residents
  • Attend meetings as required
  • Be outcome focused, positive focused and person centered focused
  • Other assigned tasks as designated by the Program Administrator.


Earn $31.50 per hour.
