Description
 ***The expected salary range for this position is based on several factors including market, funding, candidates experience level, skills, training, and related knowledge. Our ranges are developed and published as good faith estimates for the position.*** Starting Pay Range: $23.45 - $26.32 per hour Office Benefit Package: $50/Month subject to change RADIAS Health is hiring a Housing Case Manager to join our House Housing Support Program (HSP) in providing services to individuals experiencing long term homelessness who have mental illness and/or substance use disorders. The Housing Support program helps clients find their own independent housing within Ramsey County and provides supportive services to help them maintain their housing. The Housing Case Manager leads the housing search and provides support and advocacy to overcome barriers affecting housing in the past. They help clients with appointments for housing needs, accessing community resources and basic needs, crisis intervention, goal planning, and assists clients in obtaining Housing Support benefits. Housing Support Specialists will also provide instruction and monitoring of independent living skills to ensure clients have the supports necessary to manage as independently as possible in an apartment setting. Hours: Monday - Friday 8 am - 4 pm, 40 hours per week Location: This positon is a hybrid model. Some work is done in the community with clients and other work can be done from home.
