Position Purpose:



An HIM Analyst evaluates the medical record of a discharged patient to ensure that all of the proper documentation and signatures are captured for that particular encounter. Chart analysis is executed according to the departmental guidelines to ensure that medical records are completed in accordance with; The Joint Commission, Nevada State Bylaws, Renown Hospital/Facility Bylaws, regulations for patient safety, and continuing care measures. It is the responsibility of the HIM Analyst staff to evaluate chart contents and close them for further processing in a timely manner. Proper performance of the HIM Analyst results in the complete evaluation of a discharged chart in order for Coding processes to begin, generation of revenue for the organization, and proper continuity of patient care.




Nature and Scope:



This position needs to be knowledgeable of Hospital/Facility bylaws, Federal, State, Joint Commission, and other Regulatory agency requirements in relation to electronic medical record completion. It is integral that the Analysis staff identify and capture all signatures and documentation with the use of the Electronic Health Record to meet these requirements. Consistency, accuracy, promptness, and adherence to productivity standards are of paramount importance.

This candidate must be familiar with Electronic Health Records, web-based transcription systems, and electronic document/image systems in order to conduct an accurate and thorough chart analysis.

This candidate will be responsible for identifying deficits within in the medical record and will assign deficiencies to designated Health Care Providers in order to obtain any missing elements.

This position requires the demonstration of critical thinking skills and the ability to work independently.

Candidates must be familiar with HIPPA guidelines when communicating with physicians, hospital staff, and while evaluating a patient's chart. All things viewed in a patient's chart will remain confidential during the staff's analysis.

This position is responsible for developing and maintaining positive working relationships with other departments, physicians, and outside agencies. HIM Chart Analysts will collaborate with these entities on common deficiencies within a chart, such as; missing orders, insufficient documentation, patient status concerns, and any other problems that may arise in regard to chart completion.

This position should be proficient in basic release of information guidelines set forth by Renown Health, the state, and the federal government in order to correctly report the minimum amount of information necessary to assist those calling or walking-in to the department.

The candidate must consistently meet or exceed productivity and quality standards as defined by the HIM Leadership.

This position has the authority to solve problems following established company guidelines. Decisions that must be referred to a supervisor are; matters that can negatively impact the company, time-off requests, sick time, work schedules, interoffice problems, or any other issues that may arise that are not covered in the Standard of Work.

* Knowledge and understanding of computers to confidently monitor and obtain information from electronic medical records and database systems.

* Knowledge and understanding of the requirements for complete medical records, per Hospital/Facility Bylaws, The Joint Commission, Federal, State, and Regulatory body regulations.

* Ability to navigate Windows Operating System, Microsoft Office Products, Electronic Health Records Systems, Document Imaging Systems, and Office Equipment.

* Experience with Electronic Health Records and Document Imaging Systems.

Minimum Qualifications: Requirements - Required and/or Preferred



Education:



Must have working-level knowledge of the English language (including; reading, writing, and speaking English). Associates degree in Health Information Management or other related health care field preferred.



Experience:



Requires a minimum of two-year experience in a Health Information Management Department or equivalent in a health care setting. Also must possess excellent communication skills and have the ability to interact with and troubleshoot physician questions related to electronic medical record completion. Experience with Window's Operating System, EPIC, OnBase, Microsoft Applications preferred. Previous electronic medical record experience required.



License(s):



None.



Certification(s):



Registered Health Information Technician (RHIT) or Registered Health Information Administrator (RHIA) preferred.



Computer / Typing:



Must possess, or be able to obtain within 90 days, the computer skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
