Presbyterian Espanola Hospital is an acute care hospital located in Espanola, New Mexico. As a not-for-profit hospital with 80 licensed beds, Presbyterian Espanola Hospital exists to improve the health of the patients, members, and communities we serve in northern New Mexico. The people of Presbyterian Espanola Hospital -- physicians, nurses, volunteers and support staff -- take great pride in a long tradition of delivering compassionate patient care, a wide range of general acute care and specialty services to residents and visitors of northern New Mexico.

Our goal has been to provide the finest care and service possible since we first opened in 1948. Over the years, we have expanded to meet the growing needs of the Espanola Valley, Rio Arriba County, and northern New Mexico. We continue to add new services to provide state-of-the art treatment and technology for our patients.

Responsibilities:
* As a member of the PEH Addiction Medicine Consultation Service (AMCS):
* Evaluate patients in-person at PEH with substance use disorders (SUDs) as needed in the hospital units, including the Intensive Care Unit, Progressive Care Unit (Medical/Surgical unit), Labor & Delivery, Emergency Department, etc.
* If able, assist with nursery care of newborns experiencing withdrawal in collaboration with pediatricians and L&D nursing staff (if have applicable training in pediatrics)
* Communicate and collaborate with the attending physician/provider, nursing staff, and care coordination.
* Collaborate with peer support workers and behavioral health professionals (as available and as applicable).
* Order appropriate laboratory studies
* Initiate medication management for withdrawal management and initiation of long-term medication management of SUDs.
* Address co-occurring behavioral health conditions (e.g. insomnia, anxiety, depression, etc.) as able (depending upon experience and/or training)
* Prescribe applicable medications as discharge, and order outpatient injectable medications as applicable.
* Write discharge instructions related to SUD treatment.
* Write a consultation note in Epic (and follow up notes as indicated) and enter appropriate coding for the consultation.
* Consultation via telemedicine:
* Provide services as outlined above via telemedicine to other Presbyterian hospitals as possible (depending on scheduling and staffing of the PEH AMCS, ensuring that consultations needs of PEH are met).
* Provide services at outlined above to non-Presbyterian hospitals/facilities (only after such facility is fully contracted and approved by Presbyterian administration), depending on scheduling and staffing of the AMCS (and meeting obligations to PEH and other Presbyterian facilities).
* As a physician in the PEH Renewal Clinic:
* Evaluate patients with substance use disorders (SUDs) in the PEH Renewal Clinic, in person and via video (payor permitting).
* Communicate and collaborate with the nursing staff, care coordination, and patients primary care and other providers.
* Collaborate with peer support workers and behavioral health professionals (as available and as applicable).
* Order appropriate laboratory studies
* Prescribe applicable medications, and order outpatient injectable medications as applicable.
* Address co-occurring behavioral health conditions (e.g. insomnia, anxiety, depression, etc.) as able (depending upon experience and/or training)
* Provide care instructions related to SUD treatment and other conditions addressed.
* Write encounter notes in Epic and enter appropriate coding for encounters.
* Collaborate with the Renewal clinic team and leadership to treat vulnerable patient populations (e.g. housing insecure patients).
* Provide treatment for pregnant women and adolescents (as per training and experience)
* Additional outpatient services via telemedicine:
* Provide services at outlined above to non-Presbyterian clinics (only after such clinic is fully contracted and approved by Presbyterian administration), depending on scheduling and staffing of the Renewal Clinic.
* Emphasis on providing service to rural clinics in Rio Arriba County and other underserved areas of northern New Mexico.
* Provide services as outlined above via telemedicine to patients in other Presbyterian clinics (depending on scheduling and staffing of the Renewal Clinic).
* In general, physicians on the AMCS and Renewal Clinic will:
* Understand that extensive studies of much of the medication used in addiction medicine is limited.
* Strive to keep up with medical literature as well as successful practices employed in other institutions.
* Approach each patients treatment options as a risk benefit balance (e.g. some medications used to treat alcohol use disorder may be hepatotoxic, but the risks of such medication may be outweighed by the risks of continued use of alcohol).
* Follow established practices to help reduce benzodiazepine use and not prescribe short half-life benzodiazepines (e.g. alprazolam).
* Not prescribe long-term opioids except for buprenorphine (or other similar medications as appropriate).
* Keep up to date with Presbyterian assigned learning through LMS modules
