• Conducts preadmission screenings for Glancy Rehabilitation Center
• Provides support for the Intake/Discharge Coordinator as needed
• Serves as a liaison between Glancy Rehabilitation Center and physicians
• Plans and coordinates community events that represent Glancy Rehabilitation Center
• Markets the hospital system's services within the community
• Establishes and maintains effective working relationships with physicians
• Serves as an information source for community awareness of the Glancy Rehabilitation Center and the hospital system
• Participates in budget development and cost saving efforts

KNOWLEDGE SKILLS AND ABILITIES/LICENSE OR CERTIFICATION REQUIRED

1. Master of Social Work (MSW) degree and licensed as either a LCSW or a LMSW in the State of Georgia

OR

Master's degree in physical therapy or occupational therapy and licensed in the State of Georgia

1. One year clinical experience in Social Work, Care Coordination or Rehabilitation.
