Position Summary The role entails providing comprehensive psychological, social, and practical support to a designated group of patients and families. Key responsibilities include conducting social work assessments, identifying challenges, setting goals, and implementing tailored care plans. The position requires making referrals to external agencies and ensuring seamless continuity of care. Collaboration with and leadership within a multidisciplinary team is essential to enhance patient outcomes. Serving as a clinical resource expert in a specific specialty area, the incumbent will also provide clinical oversight and consultation in the absence of the Clinical Social Worker III/Director. The focus of this role is on clinical practice, education, or research, and it includes mentoring others. The position may require providing on-call coverage as needed. Competencies
Salary Range: Minimum 34.11/hour - Maximum 56.34/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
