Overview
Supports clinical operations and facilitates interactions between business support, QMS and CHHA clinical operations. Provides support to the Scheduling and Weekends, the Documentation and the Call-Handling B-Pods and CHHA Operations. Communicates and coordinates care of client as needed and in conjunction with CHHA clinical leadership. Communicates with patients and clinicians on ensuring that their needs for care, communication or records are addressed appropriately. Works under general direction. * Provides cross-regional support to all CHHA programs and other B-Pods to support call-handling, documentation and scheduling functions. Supports teams and clinical leadership by performing a critical gate-keeper role in creating clinician schedules, and triaging cases and issues to ensure the appropriate escalation to clinical management and leadership staff * Escalates issues by reporting on clinician performance and call events to CHHA clinical leadership, as needed. * Provides assistance to business staff with difficult cases by troubleshooting complaints, problems and issues to ensure accurate, effective and timely resolution. Supports staff in addressing and resolving day to day operational and clinical issues as reported via the phone. Escalates critical call issues to the attention of CHHA Clinical Managers and other relevant staff as appropriate. * Responds to calls of an emergent clinical nature and provides support related to scheduling or clinician issues as appropriate. Responds to calls and requests for patient records on both a one-off and systematic basis. Triages calls and issues to CHHA clinical manager as appropriate. Consults and collaborates with clinical managers to resolve complex issues. * Utilizes data to assess the criticality of the situation and to provide the appropriate care advice to the caller during telephone contact. * Maintains updated knowledge through continued training of IT systems and devices and of field work to be able to best assist clinicians with clinical and system related issues. * Performs documentation reviews by program and regional leadership as needed. Provides feedback on findings to support quality improvement activities /initiatives * Initiates follow-up with clinicians on the browser and other clinical work lists to ensure compliance with agency guidelines and regulation standards. Communicates clinician quality and performance concerns to team managers. * Serves as a critical source of support for all field staff and helps in addressing issues and building clinician capacity. * Supports, trains and guides BSCs and other staff as directed. * Establishes and maintains positive working relationships with CORE and CHHA leadership and facilitates communication among CORE staff and CHHA Operational staff. Establishes and maintains good relationships with internal and external parties involved in audits and record requests/ reviews. * Performs professional duties and participates in special projects/other duties, as assigned, such as provide patient care under emergent or special circumstances. * Participates in special projects and performs other duties as assigned. Qualifications Licenses and Certifications: License and current registration to practice, as a Registered Nurse in the State of New York required Education: Bachelor's Degree or Master's Degree in Nursing, or enrollment in accredited program within first year and completion of Bachelor's degree in Nursing within six years after accepting position. Work Experience: Minimum of two years experience in home health care delivery as a nurse required Knowledge of Medicare, Medicaid and other third party payers required Team leadership and group facilitation skills required Knowledge of Federal/State regulations that govern Home Care preferred Demonstrated ability to problem-solve, trouble shoot and work autonomously in a variety of clinical settings. Pay Range per Visit: If you are applying to the per diem per visit version of this job, the hiring range is as follows: $69.34- $110 per visit. Pay Range USD $119,258.00 - USD $145,458.00 /Yr.
