Activa Home Health is seeking a clinically strong PRN Physical Therapist (PT) to support our Hospital at Home Program. Provide hospital-level PT care in patients' homes focusing on mobility, balance, fall prevention, and discharge readiness. Why Join Activa's Hospital at Home Program

Job Responsibilities
* Provide skilled physical therapy services to patients receiving hospital-level care at home
* Perform comprehensive PT evaluations addressing:
* Strength, balance, endurance, and functional mobility
* Gait deviations and fall risk
* Transfers, bed mobility, and stair negotiation
* Activity tolerance and cardiopulmonary response
* Develop and implement individualized, evidence-based treatment plans aligned with acute recovery and discharge readiness
Address functional decline related to:
* Recent hospitalization
* Acute illness or injury
* Deconditioning and mobility loss
Educate patients and caregivers on:
* Safe mobility techniques and transfer strategies
* Fall prevention and home safety
* Use of assistive devices and mobility equipment
* Monitor patient progress closely, adjust plans of care, and communicate changes promptly with the care team
* Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce hospital readmissions
* Complete timely, accurate documentation in compliance with Medicare, hospital-at-home, and agency standards

Qualifications Required
* Active Physical Therapist (PT) license in the state of Florida
* Degree from an accredited Physical Therapy program (DPT preferred)
* Current CPR certification
* Valid driver's license and reliable transportation
* Strong clinical reasoning, communication, and time-management skills

Preferred / Highly Desired
Experience in one or more of the following:
* Acute care or hospital-based physical therapy
* Hospital at Home or transitional care programs
* Home health physical therapy
Skilled in:
* Gait training and balance rehabilitation
* Fall risk assessment and prevention
* Functional mobility and discharge planning
* Post-acute and medically complex patient care
* Comfortable practicing independently in a home-based acute care setting
* Familiarity with EMR documentation systems
