Details about the Case Manager position we are hiring for: Qualified applicants will need to be approved by NYS DOH Help residents achieve and maintain optimum social, mental, spiritual and emotional well-being. Serve as a resource for residents and their families. Oversee all record keeping practices and ensure they are effective and efficient.
- Collegiate degree and related experience, specifically:
- A Bachelor's or Masters' degree from an accredited college or university in Human Services, Case Management, Social Work or the equivalent, and one year of full-time experience working with the adult or geriatric population; OR
- An Associate Degree from an accredited college or university in Human Services, Case Management, Social Work, or the equivalent, and three years of full-time experience working with the adult or geriatric population.
- LPN or RN license
- Ability to work flexible hours and weekends.
- Strong organizational and time management skills.
- Ability to respond calmly to emergencies.
- Possesses good judgment, problem-solving and decision-making skills.
- Successfully complete training as required by Meridian in accordance with State regulations.
- Ability to communicate effectively with residents, families, staff, vendors and general public.
- Must have compassion for and desire to work with the elderly.
- Must demonstrate the ability to work responsibly as a team member as well as an individual.
- Must be honest, fair, dependable, respect confidentiality and the rights and privacy of others and local, state and federal laws.
- Must practice and promote Meridian Policies and Procedures, Mission Statement, Core Values and Founding Principles.
- Must meet all health requirements.
- Must pass criminal background check.
- Must be able to perform duties and responsibilities (essential job functions), with or without reasonable accommodation.
