Overview

Lumicera Health Services is seeking a Home Infusion Pharmacist to join our team!

Under the direction of the Manager, Infusion Services, the Home Infusion Pharmacist is primarily responsible for providing clinical services for patients receiving specialty infusion products, and those with bleeding disorders, primarily hemophilia. The Home Infusion Pharmacist will be responsible for significant portions of daily pharmacy operations assisting in the management of patient management programs relevant to specialty infusion medications, and communication with patients, caregivers, Hemophilia Treatment Centers (HTC) and physicians.

The Home Infusion Pharmacist will work with specialized and complex therapies applying extensive clinical skills and infusion knowledge to provide specialty infusion services administered to patients in their homes with the goal of optimizing patient outcomes, lowering emergency department (ED) visits and hospitalizations. This individual will be responsible for reviewing, and dispensing specialty infusion drugs, and clotting factors concentrates (CFCs), to support initiatives based on patient and business needs as directed by the Manager, Infusion Services. Additionally, the Home Infusion Pharmacist will work closely with other care team members including patient care assistants, clinicians, and administrative specialists to ensure the needs of the patient are met/monitored.

Responsibilities

How do I make an impact on my team?

    Review and evaluate medication orders to ensure appropriateness with consideration given to patient specific factors and medical history.

  • Prepare and dispense specialty infusion products and related supplies following state and federal regulations and the organization policies and procedures.

  • Develop comprehensive factor replacement plans, assisting with individualized perioperative plans, optimizing assay management, and actively managing medication possession ratios.

  • Perform therapeutic optimizations to improve, control and reduce treatment costs through coordinating laboratory tests, ensuring entire supply consumption, adverse event reporting, and inventory management support.

  • Develop practice guidelines to include coordination with Hemophilia Treatment Centers (HTC) and staff training.

  • Bridge the gap between hemophilia treatment centers (HTCs), the specialty pharmacy, and the patient/ caregiver by facilitating a seamless patient care transition.

  • Provide patient/ caregiver consultation on the safe use of infusion therapies, primarily clotting factors concentrates (CFCs), and disease management education over the phone.

  • Facilitate training and develop processes and procedures as new programs are implemented basing programs on evidence based on literature, data and practice standards.

  • Participating in a 24/7 on-call schedule to respond to urgent patient calls outside of regular business hours.

  • Document and report internal and external quality issues and errors to support compliance program objectives.

  • Other duties as assigned.



Qualifications

What our team expects from you?

  • Bachelor's degree in pharmacy or Pharm D.

  • Current, unrestricted licensure as a Registered Pharmacist in the state of residence, or the ability to gain reciprocity within 4 months.

  • 1-3 years' experience in a hemophilia treatment center (HTC) or a similar setting providing direct patient care required.

  • Knowledge of infusion/ specialty pharmacy medication order systems, infusion procedures and equipment, infusion products, and their uses and administration standards required.

  • Additional training in an accredited Pharmacy Residency program preferred.

  • Participate in, adhere to, and support compliance program objectives.

  • The ability to consistently interact cooperatively and respectfully with other employees.
