UVA Continuum Home Health Care is the Home Health department of UVA Health. We are an academic home health agency, providing care in the community for 30 years. The Home Care Registered Nurse is an experienced clinician who delivers hands-on care to medically complex patients across a broad range of diagnoses. In this role, you will lead an interdisciplinary home health team in collaboration with physicians and other care partners to ensure coordinated, high-quality care. You will build trusting relationships with patients and their caregivers, supporting them throughout their care journey. Responsibilities include conducting comprehensive assessments, developing individualized plans of care, implementing evidence-based interventions, and continuously monitoring patient progress to adjust care as needed in order to achieve exceptional outcomes. UVA Continuum Home Health is a department of UVA Health. We have been providing care in the community for 30 years. We serve Central Virginia, including Charlottesville, Albemarle, Augusta, Rockingham, Harrisonburg, Staunton, Waynesboro, Nelson, Buckingham, Fluvanna, Louisa, Orange, Madison, Greene and Culpeper. MINIMUM REQUIREMENTS Education: Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire. Experience: 1 year of Relevant Experience Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required. Valid Virginia Driver's License required. American Heart Association (AHA) Health Care Provider BLS certification required. PHYSICAL DEMANDS Job may require sitting for prolonged periods, frequently standing, traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease. The starting base rate for this role is $82,992.00 annually.
