JOB DESCRIPTION:

SUMMARY OF POSITION

This position will serve as a Team Leader in Military Services at Centerstone with the primary responsibilities of supervising the day-to-day operations of a team of Clinical Care Coordinators. The Team Leader will collaborate with the operational support team to assist with internal processes to maintain contract compliance and department deliverables. The Team Leader will report to the Program Manager of Military Services.

ESSENTIAL DUTIES & RESPONSIBILITIES

Staff Supervision & Development

  • Directly supervises the day-to-day work of Clinical Care Coordination team; holding regular supervision and lead team meetings to support team operations.
  • Oversee referral triage, case conferencing, documentation review, training compliance, and productivity management, ensuring compliance with contract and department requirements.
  • Provide coaching, mentorship, and professional development opportunities to staff.

Contract Compliance & Department Deliverables

  • Monitor care coordination performance metrics, completing monthly audits and submitting timely internal reports to Program Manager
  • Maintain accurate and timely case status tracking for referrals assigned to clinical care coordinators, including updates, progressions and outcomes.
  • Other administrative duties as assigned

Operational Support

  • Assists in the hiring, onboarding and training of new clinical care coordinators and operational support team.
  • Performs clinical care coordination services, including managing caseload coverage, as needed.
  • Supports the implementation of manager requests and departmental processes such as phone lines and day-to-day operational tasks.

Outreach:

  • Assists with external provider consultation and coordination, including review and processing of service continuations.
  • Serve as a point of contact for routine outreach inquiries related to services and referrals.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Growth mindset for both self and program development with drive to leverage creative solutions to complex problems/
  • A highly accountable, mature professional who listens actively and leads.
  • Self-motivated individual who can set and meet simultaneous short and long term goals.
  • Ability to catalyze change effectively in a team setting.
  • Proficiency in Microsoft Office 365 Applications, CRM Platforms, Zoom, basic Exploratory Data Analysis & Descriptive Statistics, and Adobe is essential.

QUALIFICATIONS

Education Level

Master's degree in Social Work, Psychology or related field required. Bachelor's degree with at least 3 years of leadership experience may be considered.

Years of Experience

Supervisory experience preferred; Minimum of two years' experience providing behavioral health or social services within the military community required.

Pay Range:

$43,900.00--$61,370.00
