| Job Responsibilities: |
Job Summary This position serves as the Targeted Case Management (TCM/CCM) and Community Support Program (CSP) APNP and is responsible for psychiatric care/medication prescription of TCM/CCM and CSP participants while collaborating with the program Psychiatrist. Essential Duties and Responsibilities
- Provide ongoing psychiatric evaluations of CSP participants after initial evaluation by the program Psychiatrist
- Complete initial intake for TCM/CCM participants and then provide ongoing psychiatric evaluation
- Prescribe medications and provide ongoing assertive medication management
- Pragmatically evaluates options and decides upon treatment course that will optimize safety and community success within situational limits
- Attend and participate in routine staffings with the Psychiatrist, nurses, supervisors, case managers, and other members of the CSP and TCM/CCMtreatment team while incorporating staff input regarding observations of participant behaviors and capabilities to assist with medication prescription
- Conduct clinical consultations on an as needed basis and participate in planned interventions
- Participate in treatment planning for participants as needed
- Document or dictate results of evaluations in a timely manner
- Order lab work, testing, or other procedures as needed
- Exhibit knowledge of evidence-based practices such as motivational interviewing and trauma informed care
- Assist with some nursing duties as needed
- First shift work (8am-4pm), but position has flexible hours based on availability; Thursdays are required
- Other job-related duties that may be necessary to carry out the responsibilities of the position

Required Qualifications
- Possess Chapter N 8 Certification to practice as an Advanced Practice Nurse Prescriber and registration as a Registered Nurse
- Completion of continuing education training as required by the board of nursing
- Psychiatric background is required
- No convictions or license revocations that would fail a caregiver background check
- Sufficient malpractice insurance to meet agency requirements
Knowledge, Skills and Abilities
- Good clinical skills, especially related to mental health, medical issues, and substance use disorders
- Knowledge of DSM V, psychotropic medications, and their side effects
- Ability to communicate and work effectively in a positive manner with a diverse population of staff and participants
- Ability to respond appropriately in crisis situations, including de-escalation
- Demonstrate cultural sensitivity, professionalism, and strong interpersonal skills while representing the agency both internally and externally
- Ability to manage change, think critically, and be solution-oriented when dealing with situations as they arise in a fast-paced environment
- Good organizational, dictation, and computer/typing skills
- Effective time management skills and the ability to multitask to meet various deadlines
- Commitment to the APNP role and the participants we serve
- Ability to set professional boundaries with ethics and integrity
- Work as a team member who participates productively while also managing independent duties and responsibilities
- Sense of humor

PHYSICAL DEMANDS: Position is based in an office setting with minimal driving in the community for home visits with participants; must be able to go up and down stairs and lift/carry 20 pounds; most of the work is done sitting at a desk while assessing participants, but some mobility within the office is needed. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions. WORK ENVIRONMENT: Work is performed both in a busy treatment environment, and sometimes in the community where participants' symptomatology may be evident; most of the work is done sitting at a desk; hours average 16-24 hours/week.
