Description

The Client Service Specialist is the first clinical touchpoint for Lifespark clients, families, and referral partners. This role is ideal for an experienced LPN with a background in hospice, palliative care or skilled home health who is ready to step into a highly impactful, non-bedside position that still leverages deep clinical expertise. In this role, you will use your clinical judgment, care coordination experience, and understanding of complex patient journeys to guide clients to the most appropriate services across Lifespark's continuum of care. You will assess needs beyond the surface level considering medical, emotional and social factors to ensure each client receives the right care at the right time. You will play a critical role in hospice referrals, transitions of care and high-acuity intake coordination, working closely with physicians, hospitals, case managers and interdisciplinary teams. Your experience in end-of-life care, chronic disease management, and in-home care delivery will directly influence how quickly and effectively clients access services. This position is perfect for an LPN who values relationship-building, care continuity, and clinical influence, while seeking a non-field role that still makes a meaningful difference in clients' lives.

Position: Client Service Specialist (LPN) for Hospice & Skilled Home Health

Hourly Wage: $29.00 - $33.00 hourly, depending on experience

Service Area: St. Louis Park, 55416. Hybrid schedule (office & WFH)

Schedule: 1.0 FTE, Full - Time / Monday - Friday, 8:00am-5:00pm (on-call and weekend rotation: every 6-8 weeks, holiday rotation: 1-2 per year)

Position Functions & Responsibilities:



  • Manage new client inquiries from initial contact through handoff to Case Management
  • Conduct clinically informed intake assessments, ensuring all client questions are addressed and the most appropriate service line is initiated (Home Health, Hospice, Community Care, Primary Care)
  • Receive and manage referrals via phone, fax, email, in-person, and internal back-flow calls, ensuring a seamless intake experience
  • Review incoming referrals for completeness, identify gaps, and reconcile missing clinical or administrative components through proactive follow-up with providers and care teams
  • Lead coordination of hospice referrals and transitions of care, ensuring compassionate, timely, and clinically appropriate service initiation
  • Collaborate across business lines and with physicians, hospitals, and interdisciplinary teams to support continuity of care and optimal client outcomes
  • Ensure timely service delivery in compliance with regulatory and organizational standards
  • Accurately enter and maintain referral, intake and client data in EMR and CRM systems (e.g., Salesforce) in accordance with compliance requirements
  • Drive conversion of inquiries to referrals and admissions, meeting established performance targets while maintaining a patient-centered approach
  • Use active listening and clinical insight to fully understand client needs, including complex or sensitive situations
  • Communicate clearly and compassionately with clients, families, and referral partners, adapting style, tone, and messaging to diverse audiences
  • Build trusting, long-term relationships with clients and community partners, representing Lifespark in a professional and positive manner
  • Collaborate effectively across teams, using strong interpersonal and influencing skills to align stakeholders and facilitate care decisions
  • Provide feedback and guidance in a way that motivates action and supports shared goals
  • Demonstrate professionalism, confidentiality, and alignment with Lifespark's mission and values
  • Maintain composure and effectiveness in high-volume, fast-paced, and emotionally sensitive environments
  • Prioritize tasks effectively, balancing urgent intake needs with proactive planning and relationship-building
  • Manage time efficiently, ensuring prompt follow-up on calls, referrals, and communications
  • Exercise sound judgment to focus on high-impact activities while minimizing inefficiencies
  • Demonstrate accountability by meeting deadlines, arriving prepared, and following through consistently
  • Maintain awareness of community dynamics and report insights that may impact business or client care needs



Qualifications:



  • Active LPN (Licensed Practical Nurse) license required or preferred
  • 1-2 years of experience in healthcare required
  • Experience with referral, intake, hospice, or home health strongly preferred
  • Proficiency with medical terminology
  • Strong customer service and phone skills
  • Knowledge of community resources and payer sources preferred
  • Proficient with technology (EMR, Microsoft Office, etc.)
