About the Role Optum Senior Community Care (SCC) is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care to high-risk patients in skilled nursing facilities, assisted living communities, and other senior care settings throughout Lane County, Oregon. Providers work as part of an interdisciplinary team focused on improving outcomes and coordinating care for medically complex patients.

Key Responsibilities

  • Conduct comprehensive patient assessments
  • Manage acute illnesses and chronic disease exacerbations
  • Support patients during short-term rehabilitation stays
  • Ensure accurate documentation and ICD-10 coding
  • Collaborate with interdisciplinary care teams
  • Conduct advanced illness and advance care planning discussions
  • Provide education and counseling to patients and caregivers
  • Coordinate care transitions across healthcare settings

Required Qualifications

  • NP: MSN or DNP degree with board certification (AANP or ANCC)
  • PA: Graduate of an accredited Physician Assistant program with NCCPA certification
  • Active Oregon license (or ability to obtain)
  • DEA license/prescriptive authority (or ability to obtain)
  • Ability to obtain a collaborative practice agreement if required
  • Valid driver's license and reliable transportation
  • Ability to travel to skilled nursing facilities throughout Lane County

Preferred Qualifications

  • 1+ years of clinical experience, preferably in long-term care
  • Experience caring for patients with complex medical, behavioral, or functional needs
  • Knowledge of geriatrics, chronic illness, and end-of-life care discussions
  • EMR proficiency and strong documentation skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $91,700 to $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
