The Clinical Pharmacist ensures the overall integrity, safety, rationality, and effectiveness of the drug use control process for all patients in the assigned geographic or specialty area, including the following patient groups: Neonates, Pediatrics, Adolescents, Adults, and Geriatrics. Licensure, Certification, or Registration Requirements for Hire: Current licensure with the Virginia State Board of Pharmacy required or obtained within the first 90 days of employment Licensure, Certification, or Registration Requirements for continued employment: Licensed as a Pharmacist with the Virginia Board of Pharmacy Experience REQUIRED: N/A Experience PREFERRED: Minimum of one (1) year of pharmacy practice experience in a similar setting or residency training (see below) Previous experience with state of the art drug distribution and clinical service methods Education/training REQUIRED: Bachelors of Science in Pharmacy from an accredited academic program Education/training PREFERRED: PGY1, ASHP-accredited residency PGY2, ASHP-accredited residency Doctor of Pharmacy (Pharm.D.) from an accredited academic program Independent action(s) required: The monitoring, screening, and verification of all drug-related orders for assurance of proper dosage, route, dosage form, and administration schedule. Ensure expedited preparation and dispensing of all dosage forms by pharmacy personnel. Resolve drug therapy problems. Clarify and correct problematic drug orders. Designate pharmacy technicians to perform special assignments. Evaluate employees' and students' performance. Counsel patients regarding the safe and proper use of their medications. Resolve equipment and operational problems. Interact with other health care practitioners in a multidisciplinary environment. Monitor the effects of medications, and recommend and/or initiate appropriate adjustments to achieve desired medication-related outcomes. Supervisory responsibilities (if applicable): Supervise the actions of Pharmacy Interns and Certified Pharmacy Technicians as set forth in the Pharmacy Act and the Drug Control Act with related statutes and regulations of the Virginia Board of Pharmacy. Supervise the actions of pharmacy residents as set forth in Department of Pharmacy Services. Additional position requirements: Must be available to work evenings, nights and weekends. We provide comprehensive, family-centered care for infants, children, and adolescents with a wide range of acute and complex medical needs. Our acute care pediatric services support children across multiple specialties, including general pediatrics, pediatric surgery, neurology, cardiology, hematology/oncology, and more, all delivered by a collaborative team of pediatricians, subspecialists, nurses, therapists, and pharmacists. Position Overview VCU Health is seeking a Clinical Pharmacy Specialist to join our Acute Care Pediatrics team at CHoR. This role is essential in optimizing pharmacotherapy for hospitalized children, enhancing medication safety, and partnering with interdisciplinary teams to deliver high-quality, evidence-based care. Key Responsibilities Provide comprehensive clinical pharmacy services for acute care pediatric patients, including individualized medication selection, dosing, and therapeutic monitoring. Participate in daily interdisciplinary rounds and contribute to evidence-based treatment planning across diverse pediatric conditions. Manage and optimize therapies involving high-risk medications such as antimicrobials, anticoagulants, analgesics/sedatives, and complex chronic disease regimens. Support transitions of care by ensuring safe and effective medication reconciliation, discharge counseling, and continuity planning. Educate medical staff, trainees, and families on pediatric pharmacotherapy principles and medication safety. Lead or support quality improvement initiatives, research projects, and development of clinical guidelines relevant to acute care pediatrics. Ensure adherence to hospital policies, regulatory standards, and best practices in pediatric pharmacy. Qualifications Doctor of Pharmacy (PharmD) degree from an accredited institution. Strong clinical knowledge and experience in pediatric acute care or pediatric hospital medicine. Completion of a PGY-2 residency in Pediatric Pharmacy or Pediatric Critical Care (preferred).
