Full-Time | MDS/RAI Coordinator (RN or LPN)

Schedule:

Monday- Friday

Pay Range: $39.20-53.20/ hour (based on experience)

*Same Day Pay Available*

  1. Successful completion of a State approved program in compliance with the Nurse Assistant Training Act
  2. Current certification through Minnesota Nursing Assistant Registry required.

The Difference You'll Make

  1. Coordinate and complete the full Resident Assessment Instrument (RAI)/MDS process for assigned residents
  2. Ensure assessments are completed timely and accurately in compliance with State and Federal regulations
  3. Conduct resident interviews, participate in care conferences, and attend discharge meetings as needed
  4. Apply knowledge of Case Mix, PPS, Medicare, HMOs, managed care, and third-party payors to support accurate reimbursement
  5. Utilize ICD-9/ICD-10 coding to ensure accurate documentation and care planning
  6. Review charts, admission information, and clinical documentation; identify concerns and escalate appropriately
  7. Partner with nursing and interdisciplinary teams to identify and address quality of care opportunities
  8. Support the development of individualized care plans based on resident assessments
  9. Contribute to continuous improvement efforts focused on regulatory readiness, quality outcomes, and resident satisfaction
  10. Maintain accurate records and reporting using Microsoft Office, EHR systems, ROX, and Excel
  11. Effectively prioritize workload to meet regulatory and organizational timelines
  12. Support consistent workflows and documentation standards across the clinical team

Qualifications Required

Education Required:

  1. Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing with the Minnesota Board of Nursing

Education Preferred:

  1. Certification in Gerontology

Experience Required:

  1. Minimum of three (3) years of experience in long-term care
  2. Demonstrated experience with MDS/RAI processes and Case Mix
  3. Experience working with PPS, Medicare, HMOs, managed care, and third-party payors
  4. Experience with care planning in a long-term care setting

Experience Preferred:

  1. Previous experience supporting quality improvement or regulatory readiness initiatives

Knowledge, Skills, and Abilities

  1. Strong understanding of clinical assessment processes, documentation standards, and regulatory requirements
  2. Proficiency with Microsoft Office, EHR systems, ROX, and Excel
  3. Strong written and verbal communication skills
  4. Ability to work both independently and collaboratively within interdisciplinary teams
  5. Excellent organizational, time management, and prioritization skills
  6. High level of professionalism, attention to detail, and commitment to quality care
