In keeping with our organization's goal of improving the lives of the residents we serve; the Transportation Coordinator is responsible for arranging Guest transportation ensuring that Guest appointments are accommodated ensuring guests with mobility challenges arrive to their destinations safely and on time. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. 1. Take inbound calls, emails, and other forms of communication from various service providers to schedule Guest appointments obtaining details regarding appointment requirements (i.e., Documentation and other specific items/conditions necessary) 2. Develop and maintain Guest appointment calendar. 3. Assigns Facility Driver(s) daily transportation assignments. 4. Communicate Guest appointments and appointment requirements to relevant facility departments. 5. Orient facility Drivers to the Fleet Safety Program. 6. Train and complete position specific competencies with Facility Drivers. 7. Plan, supervise, and direct the activities of Facility Drivers. 8. While acting as Facility Driver carrying out various functions including but not limited to: a. Drive Guests to their destinations on time, including clients with oxygen tanks, intravenous pumps, drips and special equipment. b. Assist Guests as they get into and out of the vehicle. c. Report back to the supervisor when Guests have been dropped off at their destination. d. Report traffic delays en route back to supervisor. e. Report any incidents or accidents to the supervisor. f. Operate wheelchair lifts as needed. g. Adhere to company policies and protocol. 9. Assure that established infection control and standard precaution practices are maintained at all times. 10. Follow established safety precautions when performing tasks and using equipment and supplies. 11. Maintains the comfort, privacy and dignity of residents and interacts with them in a manner that displays warmth, respect and promotes a caring environment. 12. Communicates and interacts effectively and tactfully with residents, visitors, families, peers, and supervisors. 13. Reports all hazardous conditions, damaged equipment, accidents/incidents, and supply issues to the supervisor. 14. Communicates and interacts effectively and tactfully with residents, visitors, families, peers, and supervisors. 15. Ensure each Guest receives person centered care. 16. Answers and responds to call lights promptly and courteously when working in resident care areas. 17. Reports all resident concerns to the appropriate department head. 18. Attend and participate in departmental meetings and in-services as directed. 19. Recognize your role as part of the Quality Assurance and Performance Improvement (QAPI) efforts of your organization. Work Environment This job operates in health care and community settings. This role requires regular walking to various locations around the facility as well as outside weather conditions while pulling or pushing residents in wheelchairs. This role also requires operating a motor vehicle on roadways, parking lots and driveways. This role may also come into contact with patients who may have contagious illnesses. Physical Demands While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping on a regular basis.
