Job Description:

Ensures psycho, social, spiritual and bereavement counseling for hospice as well as the provision of volunteers. Plays a key role in the interdisciplinary team for Home Health and Hospice.

Principle Responsibilities

  • Oversees a team of volunteer coordinators, social workers and chaplains for Home Health and Hospice.
    • Conducts ongoing coaching, assessment, training and education of staff.
    • Supports recruitment, interviewing, hiring and onboarding process.
    • Holds volunteers and employees accountable for role responsibilities, competencies, individual performance and goals.
    • Maintains scheduling, time off approval and timecard sign-off.
    • Maintains optimal staffing.
  • Oversees the work of the bereavement department that arranges support groups, counseling, community education and more.
    • Oversees Integrative Therapies Department including Massage Therapy and Music Therapy.
  • Works with computer systems to include an electronic medical record and a volunteer services system.
  • Other duties as assigned.

Required Qualifications

  • Master's degree in Social Work
  • 5+ years home care and hospice experience
  • 3+ years leadership and supervisory management experience

Licenses/Certifications

  • Licensed Social Worker - MN Board of Social Work
  • Valid Driver's License required upon hire - MN and WI residents must obtain a valid driver's license in their state of residence within 60 days of employment

Pay Range

Pay Range: $48.29 to $66.81 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience and education. Additional incentives may be available, including discretionary monetary bonuses, goal-based bonuses, and other financial incentives.
