JOB SUMMARY:

The Clinical Social Worker II or Clinical Social Worker III's (LCSW) primary responsibilities are psychosocial assessment, crisis intervention, counseling, brief therapeutic interventions, consultation, education, community linkages, supportive services, and facilitate the plan of care through the health care continuum for patients, families, and staff, with particular focus on high-risk populations. The CSWII/III acts as a consultant to the clinical team, service lines and other departments and participates in program development and quality improvement initiatives.

The Clinical Social Worker II/III will provide therapeutic support to patients and their families followed by UCSF Benioff Children's Hospital - San Francisco. The CSWII/III coordinates resources and services in response to a patient's psychosocial needs while collaborating with care team members to support timely and safe discharges and/or meet the continuum of care needs within the fiscal realities of the case. The CSW II/III will practice within the scope of UCSF's mission statement: Caring, Healing, Teaching, Discovering; and the values statement of UCSF's Everyday PRIDE: Professionalism, Respect, Integrity, Diversity, Excellence.

The CSW II/III will participate in department education and practice improvement activities. The CSW III has advanced clinical knowledge and skills and is distinguished from the CSW II by leadership activities including participation in advanced program planning, provision of educational programs, clinical supervision, and consultation to the clinical team, other departments, and the community. The CSW II/III will provide social work services to any patient regardless of gender, gender identity, sexual orientation, social, cultural, educational

or religious background, language, age, ancestry, citizenship, color, national origin, race, presence of mental or physical disability, source of payment for care, medical condition, marital status or status as a veteran.

DEPARTMENT DESCRIPTION:

At UCSF Benioff Children's Hospital, we understand that hospitalization, illness or injury can be extremely stressful. Clinical Social Workers are available in every area of the hospital to help patients and their families cope with a broad range of psychological stressors related to illness and being in the medical environment. Our Social Workers are part of the health care team to support patients through the continuum of care. We strive to reduce health disparities to increase access to care through a trauma informed and anti-racist approach.

DUTIES & ESSENTIAL JOB FUNCTIONS:

1. Provide psychosocial assessment, crisis intervention, counseling, information, referral and linkage to community referrals for patients, their families and staff.

2. Active member of the interdisciplinary team, specifically on complex psychosocial cases, contributing to team goal setting and plan of care.

3. Inpatient CSWII/III develops and implements a safe discharge plan while the outpatient CSWII/III facilitates continued care planning. Focus for both is to address the biopsychosocial needs and adjustment to illness of the patient and their family while in collaboration with the interdisciplinary team members.

4. Assess, intervene with and report domestic violence, child/elder abuse and other reportable situations in accordance with legal mandates and UCSF policy and procedure.

5. Screen for suicidal/homicidal ideation and follow legal mandates, UCSF policy and procedures to assure patient safety. Collaborate with UCSF Psychiatry, Child or Mobile Crisis and other appropriate teams on cases meeting criteria for psychiatric holds or complex behavioral health concerns.

6. Consult on challenging custody issues to clarify decision-making capacity, identify surrogate decision maker, facilitate completion of Advance Health Care Directive and/or POLST; when appropriate coordinate application for probate conservatorship or court order.

7. Knowledge and utilization of the Diagnostic and Statistical Manual (DSM).

8. Knowledge and skill with substance use and dependence assessment and intervention strategies, mental illness, end of life/bereavement, benefits and entitlements, and adjustment to illness.

9. Complies with Department standards for timely submission, accuracy and completeness of documentation in the Medical Record and collects all required statistical data.

10. Facilitates patient and family groups. Coordinate and facilitate patient and family meetings with the interdisciplinary team.

11. Attends department, hospital and community in-services related to field of practice.

12. May facilitate department, hospital and community in-services related to field of practice.

13. Participate in quality improvement activities and program planning for the Department, Hospital and Service Lines.

14. As available and under the direction of the Social Work Supervisor, CSW III's provide clinical supervision to CSW II staff for the purpose of LCSW licensure in compliance with the California Board of Behavioral Sciences (BBS).

15. Participates in coverage as needed and assigned.

16. Compliant with all UCSF policies relevant to scope of work.

OTHER FUNCTIONS AND RESPONSIBILITIES:

1. Other duties as assigned.

MINIMUM REQUIRED:

1. Master's degree from a CSWE accredited school of Social Work.

2. Current California State License of Clinical Social Work in good standing (CSW III).

3. Experience (internship/career) in a healthcare, mental health, and/or child protection working with children and families.

4. Experience in crisis de-escalation and trauma informed care.

5. Knowledge and abilities essential to the successful performance of the duties assigned to the position.

6. Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.

7. Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high-volume work environment.

8. Ability to work effectively and collaboratively with interdisciplinary teams.

9. Proficient text/pager, telephone, and computer skills including the use of the internet.

10. This position requires flexibility to orient and work at all UCSF Medical Center locations.

PREFERRED:

1. Experience working with pediatric patients, preferred.

2. Second language, Spanish.

LICENSE/CERTIFICATION:

1. Current California State License of Clinical Social Work in good standing (CSW III).

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF.
