We are seeking a part-time, 32-hour Diabetes Educator to join our outpatient Diabetes Services team. This position will provide support across multiple locations, with one day per week based in Rochester and three days per week in Dover, collaborating closely with primary care and internal medicine teams. The ideal candidate is a Registered Nurse + Certified Diabetes Educator (CDE) or Registered Dietitian + Certified Diabetes Educator (CDE) with a passion for patient education and chronic disease management. This role will work alongside and support another Diabetes Educator while partnering with providers and care teams to deliver comprehensive diabetes education and support to patients across the continuum of care. Responsible for providing comprehensive education and support to patients with diabetes, promoting self-management skills, and enhancing health outcomes. This role involves working collaboratively with healthcare teams to develop individualized care plans and facilitate diabetes management programs. Does this position require Patient Care? Yes Essential Functions: -Conduct assessments to determine the educational needs and readiness of patients to learn about diabetes management. -Develop and implement individualized education plans addressing diabetes self-management, including medication management, nutrition, physical activity, and blood glucose monitoring. -Facilitate group education classes on diabetes management, prevention, and lifestyle modifications. -Create educational materials and resources to enhance patient understanding and engagement. -Provide ongoing support and counseling to patients and their families, addressing the emotional and psychological aspects of living with diabetes. -Encourage patient adherence to treatment plans and empower them to actively participate in their health.
