The Case Manager uses a holistic approach to develop a customized care plan related to health, wellness, finances, living situation, activities of daily living, assistive technology, and life transitions. The case manager continually assesses changes in status and support needs and updates the care plan as needed. The case manager supports clients and families along the continuum of care and makes referrals and linkages with community resources. Specific vision abilities required by this job include close vision, distance vision, depth perception, and ability to adjust focus.The ability to walk up and down flights of stairs is required. Work Environment The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The noise level in the work environment is usually quiet or moderate; however, loud noise may be encountered. Job duties may be required to be performed during inclement weather and during any degree of outdoor temperatures.
