Position Overview
As a Registered Nurse (RN) at Elderwood at Ticonderoga, you'll provide direct nursing care while helping ensure each resident receives the highest quality of care possible. You'll assess resident needs, implement and monitor individualized care plans, oversee nursing assistants, administer medications and treatments, and communicate changes in condition to the appropriate members of the clinical team. Responsibilities
What You'll Do
- Provide direct nursing care to residents according to individualized plans of care
- Assess residents and monitor for changes in condition, symptoms, and overall health
- Direct and monitor nursing assistants in providing personal and nursing care
- Assist nursing assistants with resident care as needed
- Report changes in resident condition, unusual symptoms, daily needs, and progress to the appropriate nursing leadership
- Follow and help implement each resident's individualized Plan of Care
- Administer medications and treatments according to physician orders and facility policies
- Safely manage and document controlled substances and medications as required
- Complete required documentation including care plans, nursing notes, medication administration records, treatment records, and other clinical documentation
- Monitor the quality of resident care and promote adherence to established standards
- Advocate for resident rights, preferences, dignity, and individual needs
- Communicate effectively with residents, families, nursing staff, physicians, and the interdisciplinary team
- Participate in care planning, staff meetings, in-services, and other required programs
- Follow all applicable regulatory, safety, infection prevention, HIPAA, and facility requirements

Qualifications
RN - Registered Nurse Qualifications:

Credentials as a Licensed Registered Nurse (RN) with a current State license required. -
Registered Nurse (RN) Training and work experience in a geriatric setting preferred. -
Ability to supervise and work well with other personnel required. -
Ability to communicate well verbally and in writing required. -
Qualities of maturity, diplomacy, and ability to work well with others required. Caring, respectful attitude towards residents required. -
Physical stamina for constant activity required. -
Ability to perform tasks to established standards of excellence required. -
Demonstrated willingness to uphold the vision and mission of the company by following the core values of Integrity, Teamwork, and Passion for Excellence and Reverence for the Individual is required. -
This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
