Responsibilities

Position Summary:The Case Manager is responsible for coordinating safe, timely, and clinically appropriate discharge planning for patients receiving inpatient psychiatric treatment. Working collaboratively with patients, families, treatment teams, community providers, and external agencies, the Case Manager facilitates continuity of care by identifying discharge needs and barriers, coordinating referrals and aftercare services, arranging transportation, and ensuring all discharge planning activities are documented accurately and completed within established timeframes. This role requires proactive case coordination from admission through discharge to support successful patient transitions and positive treatment outcomes.


Qualifications

Education: Associate Degree required; Preferred - Bachelor's degree in Social Work, Psychology, Human Services, or a related field required.

Experience: One to two years of case management, discharge planning, behavioral health, or healthcare experience preferred. Must demonstrate effective care of coordination, referral management, documentation, and collaboration with multidisciplinary teams and community providers.

PHYSICAL REQUIREMENTS:
* Position may require walking, standing sitting for prolonged periods of time.
* Occasionally lifts up to 10 pounds and carries small objects from waist to chest.
* Requires ability to work long periods on the phone;to hear and speak clearly over the phone.
* Must be able to read and write and specifically record information and read medical records.
* Regularly needs to bend, stoop and reach.
* Emotional stability necessary to perform the essential functions of the job.
* Vision and hearing sufficient to perform the essential functions of the job.
SKILLS:
Requires organizational, verbal, interpersonal and customer service skills, grammar and spelling, the ability to read/comprehend written instructions and follow verbal instructions. Ability to establish and maintain effective working relationships with employees, managers, referral sources, patients/families, community resources, etc. Ability to exercise professional independent judgment in planning own work. Must be flexible with work hours to accommodate work volumes. Knowledge of utilization management and managed care environment is helpful.
