Summary The Pharmacy Clinical Coordinator acts as the principal point of contact for safe and appropriate medication use through education and physician liaison activities. Provides leadership to the department in the provision of clinical pharmacy services and drug information to medical, nursing, and ancillary staff. Facilitates the development, implementation, and maintenance of systems that promote optimal patient outcomes through safe, appropriate, and cost-effective medication therapy.Job Description: Position Summary The Pharmacy Clinical Coordinator acts as the principal point of contact for safe and appropriate medication use through education and physician liaison activities. Facilitates the development, implementation, and maintenance of systems that promote optimal patient outcomes through safe, appropriate, and cost-effective medication therapy. The scope of this position may include inpatient and/or ambulatory pharmacy. Minimum Requirements Graduate of an accredited College of Pharmacy. Current Kentucky Pharmacist License Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification (if applicable) or within 6 months of start date. Completion of PGY-1 ASHP-accredited Pharmacy Residency preferred or minimum of 3 years of experience in hospital practice. | |
