Job Summary: The Transitions of Care (TOC) Pharmacist is responsible for improving medication safety, continuity of care, and patient outcomes throughout the healthcare continuum. This role collaborates with physicians, nurses, care managers, case managers, ambulatory providers, and community pharmacies to ensure accurate medication reconciliation, patient education, and seamless transitions between inpatient, outpatient, and post-acute care settings.The TOC Pharmacist identifies and resolves medication-related problems, reduces avoidable readmissions, promotes adherence, and supports organizational quality and population health initiatives. Qualifications:
