Job Type: Full-Time, On-Site
Schedule: Monday-Friday, 8:00 AM-5:00 PM (occasional Medical Readiness Events and drill weekends required)

Starting salary$72.5K/yearly

About the Position

A-Team Solutions is seeking an experienced Registered Nurse Case Manager (RN) to support the Arizona Army National Guard (AZARNG) Office of the State Surgeon. This position provides non-clinical case management services designed to improve Individual Medical Readiness (IMR) and ensure Soldiers receive the support necessary to achieve deployable status. The Nurse Case Manager serves as a liaison between Soldiers, military medical providers, and unit leadership while coordinating medical readiness activities and case management programs.

Essential Duties and Responsibilities

  • Manage medical and dental readiness cases for assigned Soldiers.
  • Conduct initial and follow-up case management interviews regarding deployment-limiting conditions.
  • Coordinate treatment plans, referrals, appointments, and follow-up care.
  • Maintain and update Soldier medical readiness information in military healthcare databases, including eCase, MEDPROS, MEDCHART, MODS, eProfile, HRR, and DENCLASS.
  • Review and process medical documentation to support readiness assessments, profiling actions, Medical Evaluation Boards (MEBs), and Physical Evaluation Boards (PEBs).
  • Assist with Soldier Readiness Processing (SRP) events and Periodic Health Assessments (PHA).
  • Ensure medical records are complete, accurate, and maintained in accordance with Army regulations and HIPAA requirements.
  • Educate Soldiers on available medical, dental, VA, TRICARE, and community resources.
  • Prepare provider review packets and readiness reports.
  • Monitor medically non-deployable (MND) cases and provide weekly readiness updates.
  • Coordinate with military treatment facilities, VA providers, and other healthcare organizations.
  • Participate in readiness meetings and provide status reports and recommendations.
  • Support mobilization and deployment readiness initiatives.

Minimum Qualifications

  • Active, unrestricted Registered Nurse (RN) license.
  • Bachelor of Science in Nursing (BSN).
  • Current American Heart Association (AHA) Basic Life Support (BLS) certification.
  • Minimum of 2 years of nurse case management, care coordination, military healthcare, utilization management, population health, or related nursing experience.
  • Strong knowledge of medical documentation, case management practices, and healthcare regulations.
  • Proficiency with Microsoft Word, Excel, Outlook, and healthcare database systems.
  • Excellent communication, organization, and problem-solving skills.
  • Ability to obtain required government credentials and installation access.

Preferred Qualifications

  • Experience working with military, VA, TRICARE, or government healthcare programs.
  • Knowledge of Army medical readiness programs and deployment health requirements.
  • Experience supporting Medical Evaluation Boards (MEB) and Physical Evaluation Boards
