Clinical Coordinator, Pharmacist - RWJBarnabas Health

At RWJBarnabas Health, the Clinical Coordinator provides leadership and coordination of clinical pharmacy services across the acute-care setting. Working collaboratively with the Director and Assistant Director of Pharmacy, the Clinical Coordinator promotes safe, effective, evidence-based, and cost-conscious medication use for patients across all age groups. The position coordinates clinical pharmacy activities, supports quality assurance and performance improvement initiatives, and collaborates with physicians, nurses, pharmacy staff, and other healthcare professionals to advance medication-use practices and patient outcomes.

A typical day for a Clinical Coordinator I may include:

  • The Clinical Coordinator coordinates clinical activities of pharmacy specialists and affiliated faculty practitioners to promote consistent clinical practice and alignment with departmental priorities,
  • Coordinates the development, implementation, and assessment of pharmacist education, training, and clinical competencies.
  • The Clinical Coordinator serves as a departmental clinical resource and subject matter expert; supports the education and development of pharmacists, residents, students, and other healthcare professionals; and provides leadership for regulatory compliance and medication-use initiatives.
  • The position ensures clinical pharmacy services are provided in accordance with applicable organizational policies, accreditation standards, and federal and state laws and regulations
  • Providing integrated pharmacy services in a decentralized practice setting with providers, nurses, and other hospital personnel,
  • Conducting regular, patient-centered chart reviews, clinical assessments, and bedside rounding,
  • Developing, implementing, evaluating, and optimizing evidence-based clinical guidelines, protocols, quality improvement initiatives, research activities, and policies in collaboration with multidisciplinary patient care teams,
  • Participating as a representative on committees, system collaboratives, or service line meetings as a liaison for the pharmacy department or facility,
  • Providing leadership and support for formulary management and medication-use evaluation, incorporating evidence-based practice, medication safety, health-system alignment, clinical outcomes, and financial stewardship,
  • Providing clinical pharmacy oversight of medications in infusion pump libraries, including medication parameters, concentrations, dosing limits, and associated medication-safety initiatives
  • Developing and overseeing clinical education programs and clinical competency instruction for all pharmacists
  • Supporting the medication information education for nurses, providers, and respiratory therapists,
  • Actively participating in governance, teaching, education, and monitoring of students and pharmacy residents,
  • Direct report oversight of clinical specialists, anticoagulation trained staff, ICU trained staff, and coordination with collegiate faculty clinical specialists,
  • Providing leadership in establishing, monitoring, and improving the provision of pharmaceutical care,
  • Preparing agenda and attending meetings as appropriate including pharmacy and therapeutics committee, interdisciplinary rounds, infectious disease rounds, etc,
  • Collaborating with pharmacy leadership, operations, procurement, and clinical stakeholders to evaluate drug shortages, identify clinically appropriate alternatives, develop mitigation strategies, and communicate medication-use recommendations
  • Adhering to RWJBH Organizational competencies and standards of behavior as well as departmental specific standards of practice,
  • Providing pharmacy leadership for the Antimicrobial Stewardship Program in collaboration with infectious diseases physicians, infection prevention, microbiology, nursing, and other multidisciplinary stakeholders
  • Supporting development, implementation, monitoring, and evaluation of antimicrobial stewardship initiatives in accordance with applicable regulatory and accreditation requirements
  • Developing, monitoring, analyzing, and reporting clinical pharmacy performance metrics, medication-use outcomes, quality indicators, and regulatory measures; identifies opportunities for improvement and leads implementation of corrective or performance-improvement initiatives.
  • Serving as a pharmacy clinical resource for medication safety, regulatory and accreditation readiness, and development of medication-use policies and procedures,

To be considered for this Clinical Coordinator opportunity:

  • Graduate of an ACPE accredited College of Pharmacy or FPGEC certification and internship hours
  • Must hold and maintain an active unrestricted Pharmacist license/registration from the State of New Jersey
  • Proficiency in the use of pharmacy and other departmental software required for the position
  • BLS certification through the American Heart Association
  • Pediatric Advanced Life Support (PALS) certification
  • Advanced Cardiac Life Support (ACLS) certification within 6 months of hire
  • Board of Pharmacy Specialties certification
  • Doctor of Pharmacy or other advanced degree
