SUMMARY

In collaboration with patients/families, case managers, physicians and the interdisciplinary team, the Social Worker I provides direct psychosocial and practical care including social work assessments, problem identification, goal setting, behavioral intervention and implementation of care plans. The Social Worker collaborates with all disciplines to optimize patient outcomes; scope of services may include, but is not limited to: developing action plans to mitigate psycho-social barriers to discharge; complex post-acute care coordination; patient/family engagement and education; and employing clinical interventions (to address immediate needs.

ESSENTIAL FUNCTIONS

  • Conducts screening on all patients in assignment area with positive indicators for social work consultation.
  • Completes full psychosocial assessment and plan of care for patients and families who require intervention.
  • Provides direct psychosocial care and transition planning for a full case load of patients in assigned area.
  • Works in a collaborative practice model, facilitating care coordination with the multidisciplinary treatment team for care progression to optimize patient outcomes.
  • Adheres to departmental and regulatory documentation standards and formulates clear descriptions of the problems, goals and interventions. Adheres to timely completion of medical record documentation.
  • Maintains a current and productive network of referral resources. Seeks out local, regional and national community resources to meet patient need.
  • Assists with resource referral, capacity determination, legal guardianships, adoptions, psychiatric referral, and assessment of abuse/neglect/IPV, end of life and charity care.
  • Identifies need for and facilitates family meetings.
  • Leads or participates in interdisciplinary rounds if applicable for assignment area and articulates social work role to the team members.
  • Participates in quality improvement activities.
  • Will be required to provide coverage during absences and vacancies.
  • May serve on committees within the medical center and community.
  • Provides on-call coverage as needed.
  • May make home visits as required by assignment.
  • Participates in supervision as mandated by COMAR regulations.
  • Does not include clinical supervisory responsibility.
  • Other duties as assigned.



In addition:

Community Psychiatry Program (CPP) SW I




  • He / She Collaborates with all disciplines to optimize patient outcomes; scope of services may include, but is not limited to: developing action plans to mitigate psycho-social barriers to discharge; patient/family engagement and education; employing interventions (crisis intervention, grief counseling, suspected abuse reporting) to address immediate needs.
  • Provides direct psychotherapy and psychosocial care for a full case load of patients in assigned area.
  • Monitors effectiveness of medication and observes for and reports side-effects of medication. Notifies and consults with physician if medication changes are indicated.



QUALIFICATIONS

Education:




  • MSW required



Knowledge, Skills, and Abilities:




  • Excellent interpersonal skills, including ability to work collaboratively and cooperatively within an integrated interdisciplinary team.
  • Comprehensive knowledge of community resources.
  • Knowledge of and ability to gather relevant data to synthesize and summarize information in making judgments regarding patient care.
  • Excellent oral and written communication skills necessary to communicate/exchange and interact effectively and professionally with hospital staff, agency health care professionals, patients and families.
  • Prioritizes work in a timely and effective manner.
  • Proficiency with an electronic patient record, email, general office and communications equipment.



Required Licensure, Certification, On-going Training:




  • Maryland LMSW required



Work Experience:




  • Up to one-year experience, healthcare related field preferred
  • Field placement may be considered in work experience
