I. Major Responsibilities:
1. According to departmental care standards and protocols and all applicable hospital standards, policies, and procedures, provides clinical treatment and care management services to patients with psychiatric problems and/or dual diagnosis problems
a. Performs psychosocial assessments and develops treatment plans for assigned patients and families
b. Participates in the development of an individualized treatment plan
c. Provides goal directed clinical interventions according to defined plan
d. Initiates and oversees the discharge planning and implementation process
e. Facilitates contact and treatment team planning meetings with treatment team, family and community care providers as needed.
f. Participates in treatment team rounds.
g. Provides clinical case management back up to medical floors and Emergency Department appropriately if needed.
h. Documents all interactions in Electronic Medical Record.

2. Performs integrated and concurrent utilization, quality, and discharge planning reviews of all admissions.
a. Performs level of care evaluations and collaborates with managed care groups and insurers to determine appropriate level of care.
b. Accurately certifies admissions and continued stays using appropriate criteria.
c. Conducts timely reviews of all cases and maintains accurate files.
d. Appropriately refers cases, i.e., denials and quality issues, to attending.
e. Collects and reports data elements and clinical reviews for certification by insurers.
f. Independently works with Managed Care Providers companies. Submits required documentation when necessary.
g. Independently pursues payment for any patient that is denied coverage by a provider. Submits appeals and other requested documentation.

3. Provides counseling, education, and psychotherapy to individual patients, patient groups and patient support system members.
4. Advocates for the patient with community agencies, insurers, managed care organizations, health maintenance organizations and other providers as necessary.

Standard Staffing Level Responsibilities:
1. Complies with established departmental policies, procedures and objectives.
2. Attends variety of meetings, conferences, seminars as required or directed.
3. Demonstrates use of Quality Improvement in daily operations.
4. Complies with all health and safety regulations and requirements.
5. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
6. Maintains, regular, reliable, and predictable attendance.
7. Performs other similar and related duties as required or directed.

II. Position Qualifications:

License/Certification/Education:
Required:
1. CPI and BLS required within 30 days of hire.
2. Independent licensure as a Social Worker (LICSW) or Mental Health Counselor (LMHC) preferred or LICSW/LMHC eligible with the expectation of receiving licensure required.
3. Master's degree in Psychology, Social Work or another clinical human services discipline required

Experience/Skills:
Required:
1. Minimum two years full-time or equivalent part-time experience providing mental health and/or substance abuse assessment, treatment and/or disposition planning to acute patients preferred. Excellent verbal communication skills.
2. Ability to converse in Spanish and Portuguese is helpful.
OR equivalent combination of the above
