INTRODUCTION The Community Connector supports assigned people in a community-based day services model to promote independence and personal growth. The Community Connector will work to ensure that there is integration into the community and that each day is created to be meaningful for each person based on their desires, strengths, and needs.

KEY RESPONSIBILITIES



  1. Work with assigned individuals to implement their Individual Plan (I.P.), integrate them into the community, build skills, and develop individualized goal-orientated daily schedules*.
  2. Engage person served in meaningful community activities that assist people in developing the skills and social supports necessary to promote positive personal growth.


* 3. Inform their supervisor and other interdisciplinary team members on behavioral and other daily issues that may occur.*



  1. Provide services as outlined in Individual Plans -ensuring goals developed are appropriate, implemented, and progress benchmarks are being met.


* 5. Demonstrate positive behavioral management techniques and communicates with individuals in a person-centered, age-appropriate manner.*



  1. Complete progress notes, daily logs, timesheets, mileage logs, individual planning activities, and other documentation thoroughly, accurately, and promptly.*
  2. Facilitate an individual's self-esteem through positive interactions and building a relationship of trust.*
  3. Develop activities that promote community, self-help employment, and independent living.*
  4. Communicate with the supervisor and people supported regarding schedule implementation.*
  5. Attend and actively participate in required training, staff meetings, stakeholder team meetings, and supervision meetings.*
  6. Other duties as assigned.

SUPERVISION RECEIVED

The Assistant Director, Day Services, supervises the incumbent.

PHYSICAL QUALIFICATIONS Flexible schedule will be required due to varying hours. Must be able to stand/walk 50% of work time, sit and drive 50%. Must be able to lift, carry, push and pull 20 pounds. Must be able to stoop, kneel, crouch, crawl, and reach. All of these requirements are necessary to supervise and assist individuals with disabilities.

QUALIFICATIONS

H.S. Diploma or equivalent required and three years experience working with people with disabilities or other behavioral issues. On (1) year demonstrated ability in providing support in a respectful manner that meets the various needs of individuals served. Bachelor's degree in special education, therapeutic recreation, psychology, or related field preferred. This position will require the use of your vehicle to transport persons served. All Melwood transportation policies must be followed, and liability insurance on your vehicle must be in full force and effect while transporting for or on behalf of Melwood.
