Job Title: ELECT Case Manager Position Summary: This position works respectfully with a minimum of twenty (20) to twenty-five (25) families who are pregnant and/or parenting teens and their children, in a strengths model approach. The Case Manager will provide case management services, mobilize resources for student needs, advocate for and support pregnant/parenting teens, assist students to graduate or attain a GED, and help students improve their parenting skills. It is also the responsibility of the Case Manager to maintain student files, complete and forward all the necessary forms, attendance, and grades information, as directed by the ELECT guidelines. Duties and Responsibilities: 1. Provides support and case management for a minimum of twenty (20) to twenty-five (25) pregnant/parenting students 2. Provides resources and makes all necessary referrals to agencies and professionals. 3. Recruits and helps to retain eligible students 4. Collaborates with school nurses, school social workers, teachers, counselors, and administrators, as well as social service providers, in order to provide comprehensive support for enrolled students 5. Provides mandated ELECT services according to the ELECT guidelines including vocational/career planning, student assessment and individualized plans; instruction/information on health care; nutrition; budgeting; family planning; domestic violence and other issues that may affect pregnant and parenting teens 6. Completes home visits with enrolled students 7. Creates and maintains individual case files on all students enrolled in the program, including service plans, motivational interviews, academic records, referral, and other information as mandated by the Pennsylvania Department of Education 8. Submits monthly attendance reports, end of year paperwork, Family Works forms, and other required forms as required by PDE and the PA Department of Human Services and program administrators in a timely manner 9. Exhibits an ability to work with families, teen parents, adults, children, community agencies, and diverse groups. 10. Maintains case files for each student with clear documentation as directed. 11. Nurtures and supporting families and peers. 12. Encourages families to enroll and participate in groups and activities. 13. Helps supervise field trips and summer groups along with the ELECT Coordinator. 14. Implements and participates in weekly teen groups in partnership with the ELECT Coordinator and team 15. Establishes, advocates, and maintains relationships with providers, school districts, social service agencies, CYF, DPW, and other systems. 16. Participates in reciprocal relationships based on respect. 17. Completes paperwork in a timely fashion. Computer knowledge preferred. 18. Assists in clean-up of Center and school areas following groups and other activities 19. Works collaboratively with school district, Family Center, and other community human service providers 20. Helps coordinate information on home study/homebound for eligible pregnant/parenting teens 21. Shows computer proficiency for case notes, email, and resource/referrals 22. Communicates effectively in verbal and written form 23. Participates in weekly staff meeting, required workshops professional development, ELECT conference and any other required in-service or other training Qualifications
Bachelor's or Associate Degree in psychology, counseling, child development, social work, family relations, education, human service field, guidance and counseling strongly preferred Two years working with teens, infants, and/or children strongly preferred Experience working with parents, children, and school personnel in both home and school environments strongly preferred. Community services experience preferred. Works with diverse student populations and community and school providers Exhibits understanding and experience with the specific needs of pregnant and parenting teens strongly preferred. Ability to travel to schools for weekly groups, and to student homes for quarterly home visits, and access to an automobile required. Ability to comply with the AIU Conduct policy.
