Overview

The Social Worker (BSW) assists patients and families with complex medical and behavioral health needs to navigate uneventfully, efficiently, and as effectively as possible through the course of their illness to improve or maintain social, emotional, functional and physical health. Practices professional social work and promotes patient advocacy in accordance with the NASW Code of Ethics for Social Work. Must maintain a current Ohio Social Work license.

1.0FTE, 40 hours per week, M-F 8am - 5pm

Responsibilities

The Social Worker (BSW) functions as a member of an interdisciplinary team and provides care coordination services to patients and their families including:

    Screening and identification of patients (Occasionally)
  • Outreach to primary care physicians, specialty physicians and other service providers to identify patients in need of care coordination (Frequently)
  • Assisting Care Coordinator in completion of Health Risk Assessment (Occasionally)
  • Assisting Care Coordinator with the development of family-centered plan of care (Occasionally)
  • Facilitating communication and collaboration amongst the healthcare team (Frequently)
  • Developing patient?s/family self-management skills through education and resource provision (Continuously)
  • Performs ad hoc tasks as directed by Care Coordinator (Frequently)
  • Managing transitions between settings, caregivers and providers (Frequently)
  • Monitoring and follow-up across the continuum of care (Continuously)
  • Providing consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services (Occasionally)
  • Participates in orientation and continuing education of staff and students as appropriate (Frequently)


Qualifications

KNOWLEDGE , SKILLS AND ABILITIES REQUIRED:

1. Bachelors Degree in Social Work required.

2. Current Social Work license (LSW) in Ohio required.

3. Active BLS certification

4. Working knowledge of Medicaid and other regulatory agency standards

5. Valid driver's license & proof of auto insurance, (required to do home visits in 34 county area). Driving of personal vehicle is required as care is provided in patient homes. 6. Preferred experience working in healthcare in patient facing role
