Case Worker Outreach

Summary:
Under the supervision of the Program Manager and/or Team Leader provides case management rehabilitation and crisis interventions and outreach services in home office and community settings to adults with serious long-term mental illness. Collaborates with psychiatric and allied mental health team in assessment diagnosis intervention and treatment planning by performing the following duties. Conduct substance abuse treatment with clients in need in accordance with the 12 core functions of substance abuse counseling as defined by the ICRC.

Responsibilities:
MAIN DUTIES AND RESPONSIBILITIES:

Provides clinical interventions such as resource linking resource modification and development skill teaching and programming to consumers on Mobile Treatment Team caseload. Includes but is not limited to:

- Assists consumers in getting financial benefits;

- Assists consumers in accessing medical and dental care as need;

- Assists consumers in choosing getting and keeping employment;

- Assists consumers in choosing getting and keeping decent affordable housing;

- Assists consumers in developing social support network and participating in social and leisure activities; and

- Assists consumers in applying ADL skills in needed real-life environments.

Conducts comprehensive clinical assessment and develops interventions for substance abuse sexual abuse symptom management and family support and education.

Assesses suicidal and homicidal behavior.

Coordination of emergency psychiatric services within a timely manner including documenting interventions alternatives considered and response of client/client family and supporting agencies.

Provides coordinates and monitors treatment services for all assigned cases. Includes but is not limited to:

- 22 hours of direct services (face to face) with clients in order to adhere to minimal requirement for productivity.

- Documentation in progress notes following established procedures as described in the Individual Treatment Plan;

- Provision and coordination of care prescribed in the Treatment Plan through documented contact and implementation of assigned activities with other client/client family Gateway staff consultants and Treatment Plan-designated staff from schools and other human service and medical organizations;

Participates in treatment planning by: completing assessment assignments within time requirements specified by procedures.

Scheduling/attending treatment planning meetings with staff and other individuals specified in procedures.

Utilizes assessment information diagnostic results and staff and participation in the development of treatment plans.

Provides psycho educational services to clients and their families/significant others based on modality frequency participant and location specifications prescribed in the Individual Treatment Plan.

Services will include provision of information regarding mental illness alcohol and substance abuse effective parenting behavior management etc.

When providing therapeutic/counseling/case management services arrives and/or participates on time as scheduled; is alert and responsive to client issues and behaviors.

Works within prescriptive instructions from the client Individual Treatment Plan modifies treatment planning to meet client needs and changes in situation.

Documents with the client if possible treatment implementation and related issues in progress notes and/or treatment plan review documents.

Assists with Discharge Planning.

Medical record documentation must be in adherence to Gateway policies and procedures and MTT Medicaid guidelines: including updating of client database reviewing outside correspondence and following client confidentiality and client rights procedures.

Attends scheduled community and/or agency meetings on time.

Makes arrangements with other agencies to provide services for identified clients as specified in client Treatment Plans.

Participation in agency committees as designated by supervisor attending meetings on time and actively participating in both discussion and assignments.

Participation in other inter-agency meetings as designated by supervisor attending meetings on time and actively participating in both discussions and assignments.

Participate in daily staff organizational meetings and treatment planning review meetings.

Provide ongoing assessment of client*s mental illness symptoms and client*s response to treatment.

Provide ongoing assessment problem solving side-by-side services skill training supervision (ie assignments monitoring encouragement) and environmental adaptations to assist clients with activities of daily living.

Assist clients to find and maintain a safe affordable place to live- apartment hunting finding a roommate landlord negotiations cleaning and decorating and procuring necessities (ie utilities furniture linens).

Teach money management skills (ie budgeting and bill paying) and assist clients in accessing financial services (ie professional financial counseling emergency loan sources).

Help clients access reliable transportation (ie obtain a driver*s license and car arrange for cabs access bus lines and find rides.

Assist and support clients to have and effectively use a primary care doctor dentist etc.

Plan structure and prompt social and leisure time activities on evenings weekends and holidays.

Prepare for supervision by completing tasks assigned and formulating questions.

Monitor medication to ensure client understands prescribed dosage monitor side effects and is taken accordingly.

Attends weekly general staff meetings.

Other duties and responsibilities as assigned or needed.

Other information:

QUALIFICATION REQUIREMENTS:

Associate*s degree required. Bachelor*s degree preferred.

Two years* experience in Human Services preferred.

Experience working with individuals with substance abuse sexual abuse and/or involvement with criminal justice system is preferred.

NECESSARY SECIAL REQUIREMENTS:

Valid driver*s license and use of car during normal working hours or other means of transportation to complete major functions of job.

Automobile liability insurance coverage for vehicle being used in accordance with state regulations.
